Description
Molly Martin, DNP, ARNP, Jamestown Deputy Director of Population Health, will present a free WOW! Working on Wellness Radio Forum on Wednesday August 12th at 2:00 pm on KSQM Radio at 91.5 FM Radio. Medicare annual wellness visits are designed to help prevent disease and disability based on your current health and risk factors, and if you have been enrolled in Medicare for more than one year then you may be eligible for a medical annual wellness visit at no cost to you. These visits include a review of your medical and family history, developing or updating a list of current providers and prescriptions, detection of any cognitive impairments and advance care planning. Molly Martin is a Family Nurse Practitioner and has a doctorate in nursing. She currently serves as the Deputy Director of Population Health at Jamestown Family Health Clinic and is a care provider who oversees the diabetes department, nursing department, wellness programs, and medication assisted treatment services, and she is on the clinic leadership committee. WOW! Working on Wellness is a health education program of Dungeness Valley Health & Wellness Clinic—Sequim’s free clinic. The Clinic provides basic urgent care and chronic health care services to uninsured community members. Our work is supported by more than seventy volunteers, including physicians, other professional health care providers and laypersons, as well as private and public donations from the Sequim community and beyond. We would like to thank our 2020 Proud Supporters Home Instead, Castell Insurance and KSQM FM Radio Station. The Basic Urgent Care Clinic is open to patients on Monday and Thursday evenings beginning at 5:00 pm. Individuals interested in supporting the Clinic may call 360.582.0218.
